Use this form to exercise any of your rights over the personal data we hold about you. It is free, you do not need an account, and we respond within one month. If we need longer because the request is complex, we will tell you why before that month is up.

We will check who you are before we act. Handing someone's data to the wrong person is the worst thing we could do with it, so we may contact you to confirm your identity first. That check protects you, not us.

Withdrawing consent for your health information

If you gave us health information when you joined, you can take that permission back at any time — it is a separate thing from the requests above, and it needs no form and no reason. Tell us at the front desk, email team@chicphysiquestudio.com, or use the "Delete all my data" option above and say so in the box; any of the three works.

We delete the answers from the system straight away rather than archiving them — encrypted copies in our backups age out within 12 months — and nothing else about your membership changes. We keep a record that the consent existed and that you withdrew it — the law requires us to be able to show both — and that record holds no health information.
